The article, “Home-Based Primary Care Transforms Health Care for Medically Complex Patients” discusses the need for increasing the availability of home-based primary care and its impact on increasing quality of care and reducing costs among the nation’s costliest patients. The article goes on to discuss the unique needs of this population, as well as how HBPC differs from home care services.

It’s time again for house calls

MedStar Health, other groups work to boost doctors’ home visits

MedStar Health is joining seven medical institutions in a nationwide effort to bolster the number of doctors trained to make house calls — an outdated practice now making a comeback because it has been shown to reduce health care costs and provide better outcomes for patients.

The new training program is spearheaded by the nonprofit Home Centered Care Institute, whose mission is to increase high-quality primary care for patients too sick to leave their homes. MedStar Total Elder Care’s Medical House Call Program is being accredited as one of eight “Centers of Excellence” in major cities across the United States that will offer comprehensive training for new doctors to provide home-based primary care.

On Tuesday, the Home Centered Care Institute launched the HCCI Centers of Excellence for Home-Based Primary Care to bring high-quality primary care to more Americans. Participants include Cleveland Clinic and Mount Sinai.

The nonprofit Home Centered Care Institute works to ensure homebound patients have access to high-quality primary care in their homes nationwide. It’s establishing eight new Centers of Excellence, including MedStar’s Medical House Call Program, to offer its home-based primary care training and curriculum in major cities across the U.S.

Eight academic centers join effort to address home care shortage

About 5% of the Medicare population — or 2 million beneficiaries — would benefit from home-based primary care. These patients are too sick to leave their home and have serious chronic conditions that require frequent visits from a physician, nurse practitioner or physician assistant.

To help address the shortage, the Home Centered Care Institute along with MedStar Health and seven other academic medicine institutions launched an initiative Tuesday to train home-care providers. They estimate about 1,000 providers currently offer home-based primary care, and their goal is to increase that number to 5,000 within five years.
