HCCI Launches Nationwide Network for Home-Based Primary Care Education

JAHF grantee, The Home Centered Care Institute (HCCI), has launched Centers of Excellence (COEs) for Home-Based Primary Care with eight leading academic centers across the United States to leverage the first and only comprehensive curriculum in country to help meet the growing need for home-based primary care professionals.

On Tuesday, the Home Centered Care Institute launched the HCCI Centers of Excellence for Home-Based Primary Care to bring high-quality primary care to more Americans. Participants include Cleveland Clinic and Mount Sinai.

Inclusion of Innovative IAH Home-based Primary Care Medicare Program in CHRONIC Care Act

physician holding a clipboard for chronic care actA May 18, 2017, press release from Senator Edward J. Markey announced the Senate Finance Committee approval of the CHRONIC Care package. Among changes were the extension and expansion of the Independence at Home (IAH) Demonstration program. Specifically, the changes to the IAH program would expand the beneficiary limit to 15,000 and extended the termination date from September 30, 2017, to September 20, 2019. Also included in the changes, an IAH medical practice would have three years (instead of two under current law) to demonstrate savings before being terminated from the IAH demonstration. In response to this news, HCCI CEO Thomas Cornwell, MD stated,'”I am thankful for the continued support from Senators Markey and Wyden, and am pleased that the bill is receiving such bipartisan support. We remain optimistic that we will be able to fulfill HCCI’s mission of achieving healthcare’s Triple-Aim of developing effective, efficient approaches to home-based primary care, delivering vastly superior patient experiences, improving patient outcomes and reducing the overall cost of care.”  Read all the CHRONIC Care modifications

IAH Extended

representative peter roskamIndependence at Home (IAH), Medicare’s most successful demonstration project to date, receives stellar support from Illinois State Representative Peter Roskam.  He credits his constituent, HCCI Chief Medical Officer, Dr. Thomas Cornwell, with inspiring him to advance this important legislation. In it’s first year, IAH reported a remarkable 25 million dollar overall cost savings. Even greater savings are expected in the soon to be released second year data.
