Home-Based Primary Care – An innovative model of medical care for homebound adults – Dr. Bruce Leff

Dr. Bruce Leff is a professor of medicine at the Johns Hopkins University School of Medicine. He is an internationally recognized leader and researcher in the development, evaluation and dissemination of novel models of care for older adults, including the Hospital at Home, Guided Care, geriatric service line models and medical house call practices.

In this video he discusses the growing population of homebound adults and the use of 911 and the emergency room as their only access to medical care. He suggests home-based medical care is a method to improve outcomes, reduce hospital admissions, reduce hospital readmissions and lower costs.

The Home Centered Care Institute (HCCI) is a national non-profit organization focused on advancing home-based primary care (HBPC) to ensure that chronically ill, medically complex and homebound patients have access to high-quality care in their home.
